We understand that it is hard to precict when your child willget sick,which is why we willalways take walk-ins as long as our schedule allows. Please feel free to give us a call to schedule a same day appointment to minimize wait times

If you calland leave a message for a doctor during ofice hours, please be patient The doctor willreturn your call by the end of the work day.
o itis the patient's responsibility to understand and know thei insurance terms of coverage including, but not imited to eliibiity, Pcp assianment.
deductibles, copays, co-insurances, number of allowed well child visits per year, etc.

Patients will be responsible for all non-covered services or services applied to the insurance deductible designated by yourinsurance as "patient Responsibility" (ex, in-house lab, diagnostics labs, hearing screenings, vision screenings, etc.)

Pavment of vour copav is expected at the time of service This is reauite by your insurance We accent cash credit card HsA and FSA.

Please give us at least one to two weeks for al forms, sometimes less and sometimes more depending on the time of year. We accept or send forms via fax or email, our systems are HIPAA & HlTECH Compliant. We also alow foms dropped offin person, if you cannot drop of a form in person - you may choose to mall o any of our locations.lfyou do not wish to pick up a form in person, please give our office a callso that we can arrange a method that will suit you best.

Please alow at least 24 hour notice if you need a prescription refil or referral.lf it is not a refl,your child wil be need to be seen by one of our physicians. Pease note you can alwavs purchase certain medications such as Tvlenol, Motnin,Benadiy, clantin, zyrtec, Hvdrocorisone, and their generics over the counter without a doctor's prescription.
